Flush a bit later, to be compatible with the default value of 4096 for
PHP output_buffering, and thus avoid the need of tuning the value.
I've got the inspiration from the mark as read action. So I've used the id of the feed to trigger the action and not the url in the link. This way, it works all the time. It works in reading mode, it works in normal mode when icon are not displayed.

Now you can open the original page in the reader view with the same shortcut you'll use in the normal view.
I've changed how we identify the link to make it more flexible.
The previous way was too restrictive since the selector used a really strict path to get the url.
There was another way to achieve the same thing without changing the selector.
It was quite ugly since some meaningless class would be added on the markup to match the selector query.

The performance of lazyload.js was not good enough, and not really
needed anyway.
This change mostly affects mainly situations when the content of
articles is shown by default, not so much when they are collapsed
Using HTML5 lazyload and postpone attributes by default on all img,
audio, iframe, video.

Postpone attribute is removed by JavaScript if the user does not want
the lazyload behaviour.
In the case when users do want the lazyload behaviour, in normal view
with articles hidden, we furthermore use the data-original approach to
be sure to support current browsers.

Ajout de préfixes FreshRSS_ et Minz_ sur le modèle de SimplePie_.
Toutes les classes sont maintenant en chargement automatique (devrait
améliorer les performances en évitant de charger plein de classes
inutilisées, et faciliter la maintenance).
Suppression de set_include_path().
Si souhaité, certaines classes de Minz pourraient être déplacées dans un
sous-répertoire, par exemple les exceptions.

Ce patch supprime les htmlspecialchars qui faisaient du double-encodage,
et en modifie d'autres en entrée.
Quelques [img alt="✇" /] pour être valide, avec un caractère Unicode
représentant un film (assez proche d'un flux), mais peut-être qu'on
pourrait en trouver un mieux

Drastically reduced the number of SQL requests needed (from 233 down to
8 to load the home page with my own data set = 140 feeds in 15
categories).
Drastically reduced the amount of data transferred from MySQL to PHP.
